Some (perhaps all) find the Olympus website a bit tough to navigate.
Olympus has put together a check list on how to send back to the repair center for US customers.
If you need to send in a lens /body/accessory for repair within the USA, please refer to this document. It can save you a few dollars, in some cases 30-40% of shipping cost.

Further note, it seems that most repairs are only taking 2-3 days once the equipment arrives at the service center, very good to hear.
